چکیده

Introduction Electromagnetic stimulation is a common therapy to support bone healing in cases of avascular necrosis or pseudarthrosis. An electrostimulative screw implant (Asnis III s-series screw in Figure 1) is used to stimulate bone regeneration in the treatment of femoral head avascular necrosis. To enable positive outcome of the treatment, optimal positioning of the electric implant is critical. The aim of the present study is to optimize the implant position by an automatic modelling approach regarding optimum distribution of the electric field. Materials and Methods Optimization was accomplished using a specific software tool (iSIGHT) combined with a finite element solver (COMSOL Multiphysics®). A numerical simulation model of the proximal femur (Fig. 2) was reconstructed from pre-operative patient MRI data according to the procedure described [1]. A typical patient who suffered from avascular necrosis (Steinberg stage III C [2]) was chosen for the optimization approach of the screw implant position. The objective was to find the screw position that delivers the maximum volume of tissue activated (VTA) in the femoral head of the patient's, which was calculated by COMSOL LiveLinkTM for MATLAB®. The optimal electric field for bone tissue activation was considered between 5-70 V/m according to Kraus [3]. Results When comparing the calculated optimized screw position to the initial position derived from real patient post-operative MRI data, we found differences of 0.7 mm, 2 mm and -6.1 mm in the X, Y and Z directions respectively. The largest effect on VTA in the human femoral head was found to be due to the axial position of the screw.

The axisymmetric computational study of a femoral component to analysis the effect of titanium alloy and diameter variation.

This work presents a numerical approach in order to predict the influence of implant material stiffness in a femoral component design when submitted in compression. The implant success depends on the transferred load to the neighboring bone. The finite element method can be used to analysis the stress and strain distribution in the femoral component allowing to improve the implant selection. Fo...

Automatic implementation of a new recovery coefficient for Reliable contour milling

In contour milling, to render the machining process more automated with significant productivity without remaining material after machining, a new recovery coefficient was developed. The coefficient was inserted in the computation of contour parallel tool paths to fix the radial depth of cut in the way to ensure an optimized overlap area between the passes in the corners, without residuals. Avascular necrosis of the femoral head at 2 years after pertrochanteric fracture surgery: Case report

INTRODUCTION The avascular necrosis of the femoral head represents the death of bone tissue due to the lack of blood supply. The disease has a progressive evolution and left untreated leads to femoral head collapse and severe arthritis. CASE PRESENTATION We present a case of a pertrochanteric fracture which has been successfully operated with a dynamic interlocking trochanteric gamma nail on ...

Is the rotation of the femural head a potential initiation for cutting out? A theoretical and experimental approach

BACKGROUND Since cut-out still remains one of the major clinical challenges in the field of osteoporotic proximal femur fractures, remarkable developments have been made in improving treatment concepts. However, the mechanics of these complications have not been fully understood. We hypothesize using the experimental data and a theoretical model that a previous rotation of the femoral head due t...
